WELL-CHILD VISIT HANDOUTS

Regular checkups are crucial for your child's well-being. To maintain their health, we suggest calling our office at least 2 weeks in advance to select a date that suits your schedule best.

Vaccines and Immunizations

Vaccination Disclosure

Vaccinations are a crucial aspect of pediatric care. We strongly recommend and support vaccinations as part of the recommended immunization schedule. However, we respect the diversity of our patients and understand that some families may have religious or personal reasons to opt out of vaccinations. **

Note

We recommend you refer to Bright Futures for more information about your child  (Spanish version too).

** For non-vaccinated children, please skip this step.

Timeline checkout

2 - 5 day visit.


  • Check your baby’s weight.
  • Monitor for signs of jaundice.
  • Check to see how your baby is feeding.
  • If you need help or support with breastfeeding, please ask to schedule time with our lactation consultant in addition to your pediatrician.
  • Answer questions about your new baby.

2 week visit


  • Measure your baby's weight, length, and head size, and record it on the growth curve.
  • Check to see how your baby is feeding.
  • Answer questions you have about your new baby.
  • If you need help or support with breastfeeding, please ask to  schedule time with our lactation consultant in addition to your  pediatrician.

1 month visit


  • Measure your baby's weight, length, and head size, and record it on the growth curve.
  • Check to see how your baby is feeding.
  • Answer questions you have about your new baby.
  • If you need help or support with breastfeeding, please ask to  schedule time with our lactation consultant in addition to your  pediatrician.

2 month visit


  • Measure your baby's weight, length, and head size and record on a growth curve.
  • Administer your baby's first set of immunizations**.  Immunizations are safe, effective, and keep your child and our  community healthy. During this visit, you will be able to discuss any  questions you may have about immunizations.
  • If you need help or support with breastfeeding, please ask to schedule time with our lactation consultant in addition to your pediatrician.

4 month visit


  • Measure your baby's weight, length, and head size and record on a growth curve.
  • Administer your baby's immunizations.**
  • Review your baby's development.
  • Discuss when to start feeding your baby solid foods.

6 month visit


  • Measure your baby’s weight, length and head size and record on a growth curve.
  • Administer your baby's immunizations.**
  • Discuss you baby's development.
  • Screen your baby's eyes for any abnormalities.

9 month visit


  • Measure your baby’s weight, length and head size and record on a growth curve.
  • Review your baby's development.
  • Screen your baby's eyes for any abnormalities.
  • There are no immunizations schedule at 9 months. Speak to your provide to make sure your baby's immunizations are up-to-date.
  • Test for anemia and lead exposure with a finger prick.
  • Discuss your baby's upcoming dental care. Your baby should see a  dentist at 1 year of age.

12 month visit


  • Measure your baby’s weight, length and head size and record on a growth curve.
  • Administer your baby’s immunizations.**
  • Review your baby’s development 
  •  Discuss dental health and apply fluoride varnish to your baby’s teeth.

15 month visit


  • Measure your baby's weight, length, and head size and record on a growth curve.
  • Administer your baby's immunizations.**
  • Review your baby's development.
  • Screen your baby's eyes for any abnormalities.

18 month visit


  • Measure your child’s weight, length and head size and record on a growth curve.
  • Administer your baby's immunizations.**
  • Discuss your baby's development.
  • Screen your baby's eyes for any abnormalities.

2 year visit


  • Measure your child’s weight, length and head size and record on a growth curve.
  • There are no scheduled immunizations due at this visit. Your child may need immunizations if they are behind.
  • Test for anemia and lead exposure with a finger prick in the  office. (If your child has CHP or BCBS, the test may be subject to a fee  as your insurance does not cover this).
  • Screen your baby's eyes for any abnormalities.
  • Discuss toilet training and time out.

3 year visit


  • Measure your child’s weight and length and record on a growth curve.
  • There are no scheduled immunizations due at this visit. Your child may need immunizations if they are behind.
  • Review your child's development.
  • Screen your child's eyes for any abnormalities.
  • Preform a whisper test to screen your child's hearing.


*** For families who choose not to vaccinate their children, we encourage independent research on the benefits of vaccination through credible sources. Dr. Maria Arreaza is not obligated to discuss or challenge parents' decisions during your child's appointment. We will continue to offer the best care and support while maintaining a safe healthcare environment.
